Solve.Care’s Latest Webinar Puts Spotlight on Improving Patient Care Coordination
“A New Approach to Managing Chronic Conditions” emphasized better healthcare, better patient experience, lower healthcare costs, and higher clinician satisfaction.
Recently, Solve.Care held the webinar A New Approach to Managing Chronic Conditions, the first in a new series focused on solutions for healthcare organizations.
Hosted by Dr. David Hanekom, Solve.Care’s Chief Medical Officer and President, North America, the webinar provided attendees with insights into how emerging technologies such as distributed ledger can be leveraged to tackle inefficiencies in the current healthcare system and deliver a more patient-centric care model.
Dr. Hanekom opened the webinar with an apt quote by Dr. William James Mayo, the founder of the Mayo Clinic:
In emphasizing the message that no consideration is more important than the patient interests, this quote helped to set the tone for the rest of the discussion.
During the webinar, attendees heard from Jami Berger, the Executive Director for Operations of the Arizona Care Network, and listened as Dr. Hanekom deep-dived into the capabilities of distributed ledger technology and how it can address the issues associated with managing healthcare in larger populations. We learned about helping those who may have difficulty accessing or sourcing qualified healthcare practitioners, and the importance of helping them understand their illness.
The discussion also explored how value-based healthcare organizations can leverage distributed ledger technology to empower their patients to take control of their health journey. With a focus on chronic diseases, and more specifically the diabetes epidemic currently facing the US, Dr. Hanekom addressed how clinicians can engage their patients more by trusting them to collect and store their own data, in turn, helping them to better understand their condition.
The webinar ended with an overview of Solve.Care’s Care.Wallet and its ability to help those suffering with comorbid conditions manage them more effectively. The Care.Wallet helps patients identify where they have an opportunity to make an impact on their health through collaborating with physicians and their broader care network. Dr. Hanekom touched on the fact that healthcare is a team sport, and that a patient’s entire care circle and their level of interaction and know-how, is most effective in patient recovery.
At Solve.Care, we believe that patients belong in the center of their care. We are here simplify and enhance the relationship between them, their providers, care teams, families, friends, and everyone involved in their healthcare journeys.
